First -
During a car accident, Rei received the vivid memory of being inside of a machine that had just collapsed. She felt as though there were a man calling out her name and opening a hatch. Coming out of the memory, she found that she was being helped out by the person who was driving.
Second -
Out of desperation to reason another echo after a long dry spell, Rei decided to drink the tainted tap water, causing her to receive her plugsuit shorty after.
Third
When looking up at the strange pink lights in the sky on September 22nd, Rei's eyes turned red.
Fourth
After investigating the Tuning Towers, Rei received an echo of standing in front of a large pane of glass in some sort of testing facility. A gigantic robot pounding on the glass in front of her repeatedly, as if trying desperately to strike her.

Upon Wise Snake's emergence, Rei receives her Evangelion Unit 00.
Sixth
Being inside Unit 00's cockpit causes her Unit 00 to receive the soul that dwelled inside of it, allowing it to function.
Seventh
Seeing the pink lights over Vegas returns her the memory of battling the angel Sahaquiel.
Eighth
Her blue hair is returned after viewing the alien's press conference.
